         Grace Support Services.
Dedicated to providing residential and community support services for members of Maine communities.

Empowering members to exercise their right to choices by increasing their presence in the community through social and recreational activities.

 

Grace Support Services is a local agency, operating in Cumberland County, Maine. We provide comprehensive home and community based services for eligible members through MaineCare waiver funding (Section 21 and Section 29). Our residential facilities provide safe and supportive housing for adults living with autism, intellectual and/or developmental disabilities. Our members are supported by trained direct support professionals in their homes and communities.

Section 21 (Residential)

Grace Support Services offers 24/7 direct care supports to members living in our residential care facilities:

 

  • Personal Assistance/Mobility

  • Communication and Safety Skills

  • Nutrition/Healthy Living Skills

  • Menu Planning/Meal Preparations

  • Transportation/Community Integration

  • Medication Administration by CRMAs

  • Self Management/Interpersonal Skills

  • Achieving Person-Centered Plan/Goals

Section 29 (Non-Residential)

These services are rendered by our staff at the member's home or their perspective living arrangement:

  • Self-care/ADLs Supports

  • Independent/Safety Living Skills

  • Personal Development and Learning

  • Activities and Physical Exercise

  • Transportation to Medical Appointment 

  • Maintain Social/Community Relationships

  • Coordinate Care with Other Providers

  • Access to Spiritual/Religious activities

Mission

Grace Support Services focuses on delivering and improving the living conditions of individuals with autism spectrum disorders, and individuals with intellectual and/or developmental disabilities. We work to build opportunities, and to enable participants to make informed decisions about their daily choices. We believe members should have access to the community and resources in the community without barriers. 

Vision

Our vision is to create a society that promotes awareness and acceptance of people with disabilities, in similar fashion as we accept and dignify treatment for people without disabilities.

Goals

  • Provide a broad array of services that promotes physical, social, emotional and mental well-being of the member.

  • Equip the individual to gain knowledge and skills, to participate in his/her community in an integrated setting.

  • Provide opportunities to interact with others in the community.

  • Assist the member in achieving and maintaining a quality of life that promotes their vision of the future.

  • Assure the health and safety of all residents.

​Philosophy of Care

We envision each individual achieving important goals over his or her lifespan. People need different types and amounts of support over the course of their lives. Formal support provided through our trained professional care givers complements and supplements natural supports provided by the family and the community. We are Person-Centered; the consumers we care for are individuals who seek independence in their daily living choices. They are participating actively and seated at the center of the planning process, the decision-making process related to his or her care, and the implementation of the approved support services. We plan, coordinate, and monitor services based on this person-centered philosophy of care.

We Promote and Help Build Independence— We support and advocate for activities that empower independence and imbue confidence in our individuals in performing their daily life tasks. Toward that end, we focus on role modeling good behavior and maintain a restraint-free environment, and we seek to integrate the individuals choices, preferences, desire and needs while maintaining a safe and healthy environment.

We Support Informed Choice and Flexibility in Service Delivery— Each of us is entitled to have a broad array of choices about how we live our lives and what support looks like. Formal support is flexible and adaptable to individual needs and preferences.

 

We Support Integrated Employment— Individuals with disabilities have the right to be employed in non-segregated regular workplaces.

 

We Provide Barrier Free and Accessible Living Units in Integrated Community based Housing—individuals with disabilities should be able to decide where they live, with whom they live, when and what they eat, who visits and when, etc.
